In addition to completing the Local Authority's common application form, all applicants will need to complete the supplementary form. The information provided on this form will enable the Governors to place applicants in the correct category.

You must complete the Supplementary Form 2020/21 in addition to the application form if your child is a baptised Christian and to ensure that he/she is placed in the correct category. The Supplementary Form should be returned to school.
